In the course of our business, Côté Santé collects, uses and discloses personal information.  Your privacy is of the utmost importance to us, and we make every effort to maintain the confidentiality of the information we hold about our customers, including the personal information provided to us.

The purpose of this privacy policy (hereinafter the "Policy") is to explain our practices with respect to the collection, use and retention of your personal information.  It therefore applies to all personal information you provide to us, and by providing us with personal information, you consent to its treatment in accordance with the Policy.

Collection of personal information

In the course of our business and professional activities, we may or must collect, depending on the circumstances, several types of personal information, including :

• referring, treating or interpreting physicians, including the company's medical director;
• companies offering you benefits or compensation for our products and services (insurers, Veterans Affairs Canada, CNESST, SAAQ, NIHB, local employment centers, etc.);
• certain third parties who provide us with computer, Web and data hosting services, who may have access to the personal information we hold, strictly for the purposes of the services they provide;
• certain third parties who offer home delivery services, who may have access to the personal information we hold, strictly for the purposes of the services they provide to us;
• government authorities and regulatory bodies, including the Ordre des inhalothérapeutes du Québec, the Collège des médecins du Québec and the Ordre des Infirmières Et Infirmiers Auxiliaires du Québec, when required by law; and
• professionals providing services to our firm (legal advisors, financial advisors, insurance brokers and agents, bankers, accountants).

Your personal information may be transferred outside Quebec for the specific purposes mentioned in this Policy.

Length of time personal information is retained

Côté Santé retains personal information only as long as necessary for the purposes set out in this Policy and to comply with our legal and ethical obligations.  Thus, as of the date of the last service rendered, the personal information contained therein is archived and may be destroyed after the expiry of a period of five (5) years.

Your rights regarding your personal information

You have the right to be informed of the existence of your personal information, how it is used and whether it has been disclosed to third parties (and to obtain a list of third parties to whom personal information has been disclosed in accordance with this Policy).

Subject to the restrictions set out in the applicable laws, you also have the right to consult, upon request, the information that Côté Santé holds about you.

If any personal information held about you by Côté Santé is found to be inaccurate or incomplete, you may request that we make the appropriate corrections as soon as possible.

You also have the right to withdraw your consent to the disclosure or use of personal information held by us, to challenge our processing of your personal information and to request that we erase or destroy such personal information, subject to compliance with our legal, contractual or ethical obligations.

To exercise any right under this Policy or the law, you must address your request to the Privacy Officer as set out in section 1 of this Policy.

Safety measures implemented by Côté Santé

The personal information we collect in accordance with this policy is stored in a secure physical and technical environment. Access to personal information is limited to those Côté Santé employees who need it and who are bound by a duty of confidentiality.

Paper files containing personal information are kept in filing cabinets to which only our employees have access.  Personal information held electronically is protected by technical and technological means (identifiers - passwords, automatic backup, anti-virus software, firewalls and other technological software).

Although we are committed to maintaining a high level of protection for the personal information you provide to us, we cannot guarantee that it is completely secure, as no storage medium, physical or electronic, can offer 100% protection.
